Full-time Case Manager (Bilingual Preferred)
Meals on Wheels Central Texas is hiring a full-time Case Manager for our Supportive Team. We are seeking a person dedicated to our mission to nourish and enrich the lives of the homebound and other people in need through programs that promote dignity and independent living. This position is responsible for assessing the needs and delivering services to approximately 300 clients within their professional competency. The case manager makes appropriate referrals to both in-house and community resources and develops care plans based on the client's current situation and unmet needs. This position acts as an advocate for clients. The ideal candidate will possess the ability to work with a diverse clientele who have ongoing social service needs and skillfully assess and document clients’ needs and support network. Skills / Requirements
Bilingual (English/Spanish) preferred
Bachelor's degree in Social Work or related field (experience may be substituted)
Six months employment experience providing direct service/case management preferred
Experience with special populations preferred
Excellent writing and computer skills
Current Texas driver’s license, automobile insurance, and reliable transportation for making home visits throughout a 2-county service area
