Job Description

This position is responsible for assessing the needs and eligibility of clients for Meals on Wheels Central Texas programs and other supportive community services. As a member of the C.A.R.E. Team, you will provide intervention to facilitate optimal adjustment to situations of ongoing stress such as chronic mental or physical illness, or in acutely stressful situations such as eviction or utility disconnection. The Case Manager will conduct comprehensive assessments at client homes and make appropriate referrals to both in-house and community services, acting as advocate and service coordinator on behalf of the client. The Social Worker/Care Manager provides case management for the length of the care plan and serves as the liaison between clients, client families, caregivers, and service providers.  The position may also help clients securing financial assistance resources to facilitate housing stability.

Skills / Requirements

Bachelor's Degree in Social Work preferred or related field

2+ years of professional experience in a direct social services capacity with knowledge of case management best practices

Knowledgeable about local resources for individuals with disabilities and older adults

Reliable transportation

Fluency in Spanish preferred
